$899.00 will be charged for one Veteran Nexus Opinion Letter. A Nexus Letter is an Expert Medical Opinion that is required for a NEW service connected condition or a secondary condition for compensation benefits. A Nexus Letter provides a link for "cause and effect" to substantiate your claim. I will work in collaboration with you and your veteran service officer for submission.
899.00 will be charged for one Veteran DBQ Exam. A DBQ exam is required for an increase in service connected disability rating and compensation benefits. The DBQ exam provides an extensive in person medical and/or physical exam or a telemedicine exam if applicable that is required for an increase of your current rating of a service connected condition. I will work in collaboration with you an your veteran service officer for submission.
$1,700.00 will be charged for one Veteran Nexus Opinion Letter and one Veteran DBQ Exam. Must be related conditions. Some medical or physical conditions require an in-person exam. Refer to the Nexus and DBQ description for details.
Monthly concierge membership of $99.00 plus c/c fees to use as little or as much as you need with discounted services. An initial consultation and medical record review of $139 at the time of first visit to establish care. Dr. Lomonaco will provide primary care or assist with primary care and work in collaboration with the Veteran's VA PCP or civilian PCP. Dr. Lomonaco will provide a reasonable 24/7-365 day per year medical service for Veteran's and their families. These medical services include annual physicals with labs and labs as necessary, prescribing of medication and refill of medication, diagnostic studies, specialty referrals and everything that you would normally expect from a primary care medical provider.
