By enrolling in my month-to-month Concierge medicine services, you the client hereby acknowledge and agree to the stipulated terms regarding cancellation: To discontinue your enrollment, you must submit a written notification via email to A Doc On Wheels expressing your intent to cancel, no explanation required. Following the receipt of your cancellation request, I will terminate the processing of future payments on your account. Billing occurs on the first day of each month and your service will continue for that calendar month. When electing to purchase a membership, a service, Nexus Letter or a Disability Benefits Questionnaire (DBQ), you the client hereby provide implied and written consent to A Doc on Wheels’ for transmission and reception of your medical information and records through all technical means necessary. It is important that you are aware of and understand these terms, as agreeing to them is required.
